I encountered a problem with the online verification process.

Jul. 12, 2006 10:11 AM 		I am not able to complete my enrollment. I've been told that I exceeded my number of allowed attempts for online verification. What should I do?

Follow the instructions, and either fax the form to (585) 486-1314 or mail the form to our office. Please remember to fill out the form completely, print legibly, and include a copy of one of the accepted ID's.
